Looks a little funny but when I'm in my gym shorts I often carry wallet keys cell and Glock 34 in a fly fishing rig.

It goes on your back but it's designed to swing to the chest for a good draw-

 
It appears that I won't be able to use belts or have anything in my pockets. IWB is out. I have tried a couple of appendix holsters but just dont like them.
Have you tried using a clip that doesn't need a belt?

What is it about AIWB that you 'don't like'? Asking because in addition to better access, it's also the most comfortable and least intrusive when sitting & moving around ... IMHO. If it would work with your back issue, that Ulticlip can be worn in any position at any time - offers an opportunity to wear AIWB around the house for a few days ... just to see.

I dont have this exact belt but a stretch belt does help the nerve pain I have in my back. I have a friend who I am having try and make a leather stretch type belt for me.

 
If you are in the south, shoulder holster probably won't work. I tried it with a thin leather holster strap and it printed bad with golf shirts and button down shirts. I did get it to work with a light jacket and sweatshirt, but how often do you wear those items? Of course, it all depends on your body type. My dad bod was a no go with the shoulder holster for 90 percent of the calendar year in Florida.

Please consider ankle carry, owb, holster shirt, or off body carry. I can conceal an officer sized 1911 easily and when the back pain flares up I switch to the LCP.

After my most recent spine surgery, on number 6 now, I started carrying a cross body fanny pack. Keeps **** out of my pockets and no one thinks the guy with a pineapple on his fanny pack is carrying a gun.

This is true. If you're wearing your belt tight and the pressure on your lower bsck is causing problems, get suspenders and wear your belt looser. It'll take the pressure off your lower back and posdibly aliviate your problem. You just have to get used to wearing sudpenders
I recently acquired a "Kore Essentials Gun Belt" and love it. Quick 1/4" adjustments as needed helped my back problems.
 
Another vote for trying something like the Hunter Constantine belt (Tier 1 makes a version as well) with a quality, purpose-built AIWB holster. If you haven’t tried one already, an AIWB-specific holster from JMCK, Dark Star, or PHLster plus the right belt can make all the difference and take AIWB from “nope, not happening” to “I’ll never carry any other way again.” Be sure to try a foam wedge, especially if your stomach isn’t perfectly flat.

I don’t have chronic back problems but carrying with other belts, even good ones, did put enough pressure on my back to be uncomfortable. The Constantine is perfect as it’s stiff enough up front to support a 1911 but at my back it feels like I’m not wearing a belt at all.

Some people have good luck using the Constantine under their pants and a tucked shirt like a PHLster Enigma, which may be useful for your office setting. That doesn’t work for me personally because with my build plus the clothes I wear the Constantine is just a little too bulky up front, but I can see how it works for others.

I was thinking about suspenders. Never tried those that go under the shirt, but i will likely try them. Thank you
 
Ken Null Holsters makes an under shirt (not undershirt) shoulder holster. Minimalist. Take a look. I have one, unused, called the "city slicker" that he no longer makes. It is specifically for a S&W centennial j-frame. If the free ones from the other gentleman don't work out, send me a pm.

I am carrying appendix today. It does take pressure off my back. It is fine under normal circumstances, but I work in a very public place and am always in front of people. Sooner or later I am going to raise my arm high enough during a presentation to show it. I have bought larger shirts recently, but the fear is still there. It's too hot to wear a sport coat all the time too. That would probably the issue

The rear sight on my lightest handgun is also sharp and digs in sometimes. I wish I had kept my G43. It would work well this way

I was using a KORE belt which was terrible for me as all it did was hula hoop and place more pressure on my hips without pulling the holster closer to my body. With this belt I now carry a GPArms Patriot without back issues.

Anything that can't keep the gun as close to you as possible will probably cause issues.

Having recently retired from working S Florida restaurants - bending, sitting, stooping & much reaching - I really do get it. But I tried duplicating your reaching concerns but with no luck. Not knowing your height nor your EDC, consider these:
A) Try tall men's shirts
B) Bellyband worn low, below the belt, with pistol at 12:00
C) Kydex trigger guard 'holster' with paracord - loop cord around pant button then button & zip up and drop pistol straight down at 12:00.

I've worn both B & C at nonpermissive environments with great success, too.

Is there something like a shoulder holster that goes under the shirt? I work in an office setting, and live in the south. I dont wear long sleeves till January. But my back is giving me fits. It appears that I won't be able to use belts or have anything in my pockets. IWB is out. I have tried a couple of appendix holsters but just dont like them.

Are there any other options?
Fused at L4 to S1. On days it bothers me I carry a SW 442 in my front pocket.

I feel your pain, literally.
